Eternal Sunshine Of The Spotless Mind was released ten years ago today, and we're all still so alone, and cold, on a beach in Montauk in the wintertime, haunted by faded memories that never really got exorcised, and that Beck song...
Lacuna, Inc. still isn't real... but you can find traces of it, and other pieces of the movie, around New York—from Montauk and Yonkers to the LES and Williamsburg.
504 Grand Street on the Lower East Side (Lacuna, Inc.—watch Dr. Mierzwiak's commercial here)

Franklin D Roosevelt State Park, 2957 Crompond Road, Yorktown Heights (This is where the frozen lake was! Recreate the photo, it's about 40 miles from New York City)

59 Orient Avenue (Clementine's apartment is here. IRL it was recently renovated and rented out after being abandoned... it's also down the block from Michel Gondry's home)

123 Valentine Lane, Yonkers (Joel's apartment complex)

180 3rd Avenue (Dang Lai Palace, where Joel and Clementine have a fight—"Dinner at Kangs again...")
Orchard Street at Rivington (the happy couple takes a stroll around the neighborhood)

Grand Central Terminal

Columbia University, Broadway & 116th Street (The bookstore where Clementine works, and where she doesn't recognize Joel)
And here's a map with many more locations:
